Requisition form for resources from UGA

Reagents on this page require current FR3 registration and a University of Georgia material transfer agreement.

 

When ordering parasites, please use the boxes to indicate the NUMBER of parasites you want in your order.

Please use the boxes to specify the quantity of worms, mosquitoes, or eggs you require; and check the appropriate buttons to indicate whether you want them shipped live or frozen. Also, please realize that because of budget constraints, we do not keep a large number of infected animals in house. If your needs for biological materials exceed the alloted limits, please contact eburkman@uga.edu so we can coordinate your requests.

Please realize that because of budget constraints, we do not keep a large number of infected animals in house. Requests for infected animals must be made several months in advance, and by direct contact with Mike Dzimianski and Erica Burkman (the following link sends to both addresses)  Mike Dzimianski.  The user MUST provide a desired shipping date, and is responsible for maintaining an account with an authorized animal carrier (i.e. DB Schenker (formerly *Bax* *Global*, *Inc*.), World Courier, or UPS Supply Chain Solutions).  The user will be responsible for scheduling their own shipment and for all costs associated with the shipping of these materials.  The user must also provide the account information to parasite resources.  If you have any questions about this process please contact either Mike or Erica
