Requisition

Options for online or print ordering from Smith

To order Molecular Resource reagents, it is necessary to have a Material Transfer Agreement for Smith College.

 

To request DNA libraries, individual clones or other reagents for your filariasis research, you can:

  1. Submit a request online

  2. Print the form, fill it in, and mail or fax it to the following address:

cDNA and genomic libraries are shipped in dry ice (UN1845). The bacterial libraries are sent as a stab culture at ambient temperature.  The human serum and Bm14 antigen are provided by the FRRRC.

Shipping within the U.S. is through Federal Express (FedEx) or DHL.  Shipping to other countries can only be done via FedEx or DHL because they are the only carriers that allows us to ship dry ice and dangerous goods shipments.

To expedite your order, please be sure to include the information found on the following check list:

  1. Your name
  2. The name of your institution
  3. Your shipping address
  4. Phone and fax numbers if available
  5. Email address
  6. The name of the Principal Investigator of the laboratory that will receive the materials
  7. For shipments, your institute’s Federal Express account number that can be used for shipping (5.0 lbs dry ice).  Some institutions will be required to make advanced arrangements with FedEx for shipping.  Call your FedEx representative for information before you place the order.  Check your country's regulations on imports for permit requirements on infectious substances and biological substance.
